CONCLUSION OF ANNUAL CONTRACTS FOR HEALTH CARE SERVICE PROVISION

Next yearSI “Republican Clinical Medical Center” of the Administration of the President of the Republic of Belarus continues the provision of services under direct contracts for fee-based provision of health care services as part of Comprehensive Health Care (CHC) and Outpatient-Polyclinic Health Care (OPHC) programs.

The contracting campaign for the year 2022 starts on 01.11.2021. You may discuss any contractual issues with Yefim Semyonovich Khavkin by telephone at the numbers (017)5434229.

Patients intending to conclude a comprehensive health care contract or an outpatient-polyclinic health care contract that were not previously serviced by the Center under long-term contracts, as well as those who have not requested medical assistance for over 9 months shall provide the following documents:

List of documents

• Выписка из медицинских документов (форма №1мед/у-10) из поликлиники по месту жительства, содержащую информацию
  • обращения за медицинской помощью в течение последних 5-ти лет с уточненными диагнозами и результатами ежегодной диспансеризации в течение последнего года;
  • результат рентгенофлюорографического обследования органов грудной клетки в течение последнего года;
  • результат осмотра врачом-гинекологом в течение последнего года (для женщин);
  • сведения о плановых (экстренных - если проводились) профилактических прививках от дифтерии и столбняка.
• Справка из психоневрологического диспансера (состоит или не состоит пациент на учете)
• Справка из наркологического диспансера (состоит или не состоит пациент на учете)

PATIENT ADMISSION AND CONSULTATION

Health care services, including fee-based services to patients not assigned to SI “Republican Clinical Medical Center” of the Administration of the President of the Republic of Belarus, shall be provided as follows (by service directions):

1. Surgery.
   Pre-admission consultations will be carried out based on previous appointment, on Mondays, Wednesdays and Fridays at 13:00 till 14:00, at office Г 112 (turn right from the central entrance).
2. Gynecology.
   Pre-admission consultations shall be carried out based on previous appointment, on Tuesdays and Thursdays at 14:00 till 15:00, at office Г 112 (turn right from the central entrance).
   
3. Urinology.
   Pre-admission consultations shall be carried out based on previous appointment, on Mondays, Wednesdays and Fridays at 14:00 till 15:00, at office Г 112 (turn right from the central entrance).
4. Cardiovascular Surgery.
   Pre-admission consultations shall be carried out based on previous appointment, on Mondays and Wednesdays at 15:00 till 16:00, at office Г 112 (turn right from the central entrance).
5. X-ray endovascular surgery and interventional electrophysiology.
   Pre-admission consultations shall be carried out based on previous appointment, on Tuesdays and Thursdays at 13:00 till 14:00, at office Г 112 (turn right from the central entrance).

• Head of the Cardiovascular Surgery Department Kozlov O.I. : Mondays; 15:00 – 16:00
• Head of the Gynecology Department Kazakevich А.I.: Mondays and Thursdays 14.00 – 15.00
• Head of the X-ray Endovascular Surgery Department Postoyalko А.S.: Tuesdays and Thursdays 13.00 – 14.00
